Situation-
, this is Martha from the Pond Placid Emergency Room. I am calling about your patient, Mrs. Z, whom came in this afternoon complaining of minor schwindel, weakness, and blurry eye-sight, as well as an injured hip.
Mrs. Z is 79 years old, 5′ 10″ and weighed in at 161 pounds. She actually is a retired teacher who has lived alone since the fatality of her husband two years ago. Mrs. Z was brought into the clinic by a neighbor, since she gave up driving of a year ago saying the lady had difficulty seeing in strong sun rays and in the dark.
Mrs. Z was admitted in the ER after a fall that injured her left hip. Background-
1 . Client arrived after slipping and wounding her still left hip. Consumer indicated that she was feeding the community cats and did not view the steps while she was walking. Even more, she confessed that the girl might have acquired several falls both inside and outside the home over the last several months.
2 . Client is fairly active, in very good psychological wellness, knows being used to drive. Your woman occasionally self-medicates with six hundred mg Ibuprofen for osteo issues. Consumer does not use tobacco, liquor, or illicit drugs and presents not any emotional or perhaps psychological concerns.
3. Were not offered client’s current eyeglass prescription. Her vision may be going down hill, and there are a lot of indications of cataracts: clouded or blurry vision, problems with evening vision, level of sensitivity to glare. Falls do not appear to be due to anything neurologic or cardiological, simply client being unable to observe appropriately (Cataracts, 2012).
Assessment
1 . Stress is unremarkable, and customer reports zero unusual symptoms except for dropping. All immunizations are up dated, and no difficulties reported by recent surgical procedure. Client reveals to be well balanced, with no indicator of robust concerns. There is no indicator of cerebrovascular accident or different neurological concerns. (D’Amico Barbarito, 2011).
